Initial Consultation

First Visit

Your initial appointment will consist of a consultation. This is time for us to complete a clinical examination and review pertinent x-rays and lab tests if needed. Time is taken to understand your needs, explain the proposed treatment and answer your questions. We want you to be well informed about your diagnosis and treatment options. Occasionally, surgery can be done on the same day as the consultation. However, a complex medical history, complex treatment plan, or the need for intravenous (IV) anesthesia will require a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip from your doctor and any x-rays if applicable. If your doctor has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are needed, they can be obtained at our facility.
  • A list of any medications you are presently taking.
  • If you have medical or dental insurance, please bring the necessary information. This will save time and allow us to help you process any claims. A pre-operative consultation and physical examination is mandatory for patients undergoing I.V. anesthesia for surgery.

IMPORTANT: All patients under the age of 18, must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery. (i.e. diabetes, high blood pressure, artificial heart valves and joints, history of rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

We have made a commitment to work with your doctor to provide you with the highest quality of care. Because we recognize the importance of good communication between doctors, we make every effort to ensure that your family dentist is informed about all aspects of your treatment.
